These are birth certificates that are registered with the office a year or more after the date of birth. There are two types: certificates filed by individuals and court-ordered. The information on the certificate includes the individual's name, sex, date of birth, and place of birth; the parents' name, race, and birthplace; the signature of the registrant or applicant; supporting documentation of the facts of the birth; and the registrar's certification.

Many citizens needed to file a delayed certificate of birth because their birth was not attended or not registered. In July 1941, the Division of Vital Statistics received approval from the State Board of Health to establish a "sub-division of delayed registration of births." The division director, Eva W. Ramsey, conveyed to the Board that the "present national emergency has made it mandatory that all persons employed in defense industries present a birth certificate as proof of citizenship," yet Utah had none before 1905. Specific forms were devised based on the U.S. Bureau of the Census (Board of Health Minutes, Series 2565). A form of the current statutory language in Utah Code 26B-8-108 first appeared in 1953 under "Belated or Delayed Filings."

Records using the new form begin in November 1941. A scattering of other belated birth certificates going back to 1916 were also gathered at some point by the office, comprising the first half of Box 1. Additional delayed registrations may be found in the standard birth certificates (Series 81448), kept where a birth could have been filed originally.

Unlike certificates registered at the time of birth, delayed registrations do not contain information on the complications at birth nor useful health information to improve birth conditions. To receive a delayed certificate, three sources of proof of birth are submitted by the applicant: two documentary proofs such as a church record, family Bible, or from the census, plus an affidavit from an older relative present at the birth. The State Registrar determines whether enough proof has been provided to issue a delayed certificate of birth. In a few cases, these document proofs have been kept with the certificates.
