DESCRIPTION: These files document applications, registrations, and licenses for the construction trade industry. The Department of Commerce, Division of Occupational and Professional Licensing, issues licenses to qualified applicants. Applicants are required to submit an application, pay a fee, and pass an examination approved by the Contractors Licensing Board, the Plumbers Licensing Board, or the Electricians Licensing Board UCA_58-55-5_(1991). Upon approval of the license, applicants are required to file proof of workman's compensation and public liability insurance, proof of registration with the Department of Commerce, Divisions of Corporations and Commercial Code, and proof of registration with the Department of Employment Security and Utah State Tax Commission. Applicants are required to renew their licenses biennially UCA 58-55-6 (1991). Information includes name, address, telephone number, social security number, birthdate. Also, business name and the date operations are commenced are included.
