DESCRIPTION: These files document applications, registrations, and licenses for institutional pharmacies. The Department of Commerce, Division of Occupational Licensing, issues licenses to institutional pharmacies that have met the requirements for licensure under the laws of the State of Utah. Applicants are required to submit an application approved by the State Board of Pharmacy, pay a fee determined by the Department of Commerce, adhere to minimum standards for operations and employee conduct, and maintain prescription files and other records in accordance with applicable state and federal regulations (UCA 58-17b-303 (2005). Information includes name, social security number, mailing and business address, and endorsements.
