DESCRIPTION: This system documents employees' insurance coverage. It is used to collect data to transfer plan selection and coverage data to the insurance company for enrollment purposes, to calculate premiums owed by the district and its employees, and to assist with claims processing or for reference purposes. The system includes the following employee information: name, social security number, date of birth, insurance plan(s) selected, dependent information, eligibility information, life insurance beneficiary designation, a history of coverage and plans including effective dates.
