DESCRIPTION: Closed medical charts including correspondence, WIC voucher copies, WIC program certification and termination form, medical history, and nutrition history for adults in conjunction with the Women Infant Children (WIC) program to determine program eligibility of client. The chart indicates enrollment status in the program, nutritional requirements, and documents medical services received by the mother before and after the birth of the child.
