DESCRIPTION: This monthly report is sent by the State Division of Corporations and Commercial Civil Code to notify county clerks of notaries commissioned during the previous month. County clerks are required to "keep and maintain an indexed record" of all notaries within the county" (UCA 17-20-3 (1993)). The report includes month, date, notary public's name, address, commission date, and date commission expires.
