DESCRIPTION: These surveys are used by the division to identify health concerns of high school students. The survey indicates health behavior disease risk factors, and the attitudes of these students so the Health Department can create and plan better health education programs and evaluate the effectiveness of current programs. Information includes general information, such as, age, height, race, gender, family health histories, and individuals previous health care. Also includes student life style information, such as, smoking, drinking, eating habits, and religion.
