DESCRIPTION: Computer data files and paper reports created to track complaints,investigations, and licensing board decisions against licensees engaged in unlawful activities. -Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
--Purpose Information is Collected: The Division collects PII information in this series to assist the Division in its investigation and review process.
