ARRANGEMENT: Alphabetical by surname, thereunder numerical by investigative case number.
DESCRIPTION: This series contains complaint files created for the purpose of documenting unlawful or unprofessional conduct. If sufficient evidence of unprofessional or unlawful conduct is discovered, the Division initiates a formal investigation under UCA 58-1-106(1)(b). -Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
