DESCRIPTION: These case files document the participation of children in the Child Health Evaluation and Care Program (CHEC). This is a low cost program to identify public health problems and to promote healthy children in low income families ineligible for Medicaid. These files are also used to document participation in the program and to record the children's development. They include the child health record, well child care 4-5 years of age form, and well child care examination forms. These records are also called Well Child case files.
