DESCRIPTION: These records support the agency's function to document and certify the facts of deaths for the legal purposes of the state's citizens. Records document reports submitted to the agency listing all deaths handled in the state for the month (Utah Code 26-2-18 (2006); 26-2-16 (2009); 26-2-23 (2009)). Records are used by the agency to verify that the number of deaths reported is equal to the number of death certificates received. Information includes contact information for the reporting entity as well as information about the deceased. Information may also include the location and cause of death, disposition of remains, and location of interment. Schedule includes monthly reports from cemeteries, funeral directors, and heath care institutions.
