DESCRIPTION: These records provide notice to the Driver's License Division from insurance companies that an individual's auto liability insurance policy has been terminated. Records may include name, address, license number, birth date, case number, social security number, insurance company name, policy number, effective date of policy, termination date of policy and dated signature of an insurance company representative.
