DESCRIPTION: This is a record of the provider, the program, and the services offered to clients. Providers are audited by the division for compliance of state and federal codes, as well as division requirements. These records can be used for litigation. This series includes complaints of abuse, accusation of misappropriation of funds, interviews with house parents, accusations of medical and dental neglect, prejudicial and preferential treatment, unqualified staff, Confidentiality Agreement, Rationale for the Cancellation of the Contract, Problem Correction Plan, Fact Finding Report, Results of Report, interview notes, contract, contract guidelines, program outline, and related correspondence.

FORMAT MANAGEMENT
Paper: Retain in Office for 3 years after contract is terminated or until contract expires and is not renewed and then transfer to State Records Center. Retain in State Records Center for 5 years and then destroy provided no litigation is pending.
