DESCRIPTION: These are reports of adverse action taken against licensees for violations of regulations or laws. The Department of Commerce takes disciplinary action against violators and also sends the information to various professional associations. -Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
