DESCRIPTION: This record series documents the participation of licensees in UPHP as outlined in Rule R156-4a. It includes records related to screening, intake, assessment, treatment, monitoring, and compliance for licensees who have been determined eligible and have enrolled in the program. This record series may include, but is not limited to: referral, screening and intake documentation, comprehensive clinical evaluation reports, treatment plans, progress reports from employers, treatment and medical and mental health providers, toxicology test results (urine, blood, breath, hair, nail, or tissue samples), compliance documentation (e.g., meeting attendance, progress on treatment plan goals), case management notes, correspondence with the licensee, healthcare providers, and other relevant parties, and any documentation related to program participation, completion or termination.
Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home and mailing addresses, email address, phone number, criminal history, employer contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
Purpose Information is collected: UPHP collects information to monitor licensee compliance with program requirements, track progress in treatment, and ensure the safety and well-being of the public.
