DESCRIPTION: These electronic records are created for the purpose of tracking investigations and complaints dealt with by the Division of Occupational and Professional Licensing. Information includes name, address, social security number, and results of investigations. -Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
