DESCRIPTION: These files document client history of individuals seeking medical services through the Workforce Services Employment Centers. Client information is used to determine eligibility for the following programs: Medicaid; Utah Medical Assistance Program, UMAP; Child Health Insurance Program, CHIP; and Qualified Medical Beneficiary, QMB. Information includes the individual's income, asset availability, place of residence, social security number, birth certificate, paternity information, insurance coverage, and medical disability status. The files are initially generated by employees of the Health Department who are located in a Workforce Service Employment Center. After which they are shared between the Health Department and Workforce Services. Workforce Services has assumed the responsibility for managing the files which are physically located in the Employment Centers.
