DESCRIPTION: These files were created to establish eligibility for the Utah Women, Infants and Children (WIC) program. The information includes medical history, diet history, prenatal weight gain and growth grid, nutrition education contact sheet, prenatal survey of infant feeding, and progress notes.

The records were created by the Provo Migrant Clinic, which administered a WIC program. The Provo clinic closed, necessitating transfer of the records to the Utah WIC office. Other WIC clinics may access these case files, if needed, to determine eligibility.
