DESCRIPTION: These case files document the client history of any individual who applies for help or receives services through the Bureau of Eligibility Services. Client information is used to determine eligibility for the following programs: Medicaid, Utah Medical Assistance Program (UMAP), Child Health Insurance Program (CHIP), and Qualified Medical Beneficiary (QMB). The case file is also used by the Department of Workforce Services if clients apply for food stamps, general assistance, or child care services. The agency verifies an individual's income, asset availability, place of residence, social security number, birth certificate, paternity information, insurance coverage, and medical disability status.
