DESCRIPTION: Regulated controlled substance distributors and purchasers are required to submit quarterly reports to the Division of Professional Licensing regarding chemical precursors that are sold or purchased. These reports include purchaser names, their D.O.P.L. license numbers, and the chemicals being purchased. The Division, as well as law enforcement agencies, use the information collected to track users of controlled substance precursors. This information may be used in criminal or administrative legal action. Personal Identifying Information (PII) that may be contained within this records series consists of: name, username, email address, social security number, date of birth, drivers license number, home/mailing address, medical records, IP address, and biometric data
