DESCRIPTION: These are forms submitted to the agency by public water suppliers to document the chlorination levels of the public water supply. The information is used to verify whether water systems are in compliance with state and federal regulations. The forms are submitted on a monthly basis. Information includes the daily readings for chlorine residue, water meter totalizer, and water volume; dates and times of testing; and signature of person submitting information.
