DESCRIPTION: These are non-disciplinary stipulation and consent orders allowing companies to engage in the filling of internet pharmaceutical perscriptions. Also included are documents demonstrating compliance with stipulation and consent orders. Information includes name of company, agency case number, date of order, and signatures of representatives of consenting parties. -Personal Identifying Information (PII) that may be included in this record series consists of: name, email address, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
