TITLE: Child care subsidy history files

DESCRIPTION: The Office of Child Care maintains market rate studies, income eligibility and copayment tables, subsidy payment tables, needs assessments and audits of child care subsidy programs. The Utah Child Care and Development Fund (CCDF) state plans are also available, which include program requirements.

Purpose personal data is collected, maintained, or used: Child care providers business contact information and banking information is collected so that subsidy payments can be directly deposited into their account. Child care providers provide their federal tax identification number so that income can be reported accurately on an IRS 1099 form. Customer information is provided so that child care providers can certify the child is enrolled and met minimum attendance requirements and to report any custom rates for customers' child(ren) below the Department's set subsidy rate for the age of the child.
Legal authority for collecting, keeping, or using personal data: Utah Code 35A-3-2-209 Award of child care subsidy services, and CCDF Regulations: 45 CFR 98.
