DESCRIPTION: These records contain registration information provided by individuals seeking access to the state's controlled substance database. Information is collected to verify and authorize user access to the database. Data collected includes name, birth date, address, e-mail address, Drug Enforcement Administration (DEA) registration number, driver license number, law enforcement originating agency identifier, medical/dental information, state occupational license number, telephone number, and user identification number. -Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, medical/prescription records.

-Purpose Information is Collected: The Division collects this information to be in compliance with statute/rule requirements relating to the Utah Controlled Substance Database.

-Statute citation reference to collect PII information: Section 58-1-301 and Title 58, Chapter 37
