DESCRIPTION: These records contain requests received from authorized persons for information in the controlled substance database. The files contain the name, address, and phone number of the requestor. They also contain identifying information of the persons about whom information is being requested such as: names, dates of birth, social security numbers, and signatures.

Under U.C.A. § 63A-19-401.1 (2025), the Utah Department of Commerce hereby discloses this record series includes personal identifying information.

Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, medical/prescription records.

Purpose Information is Collected: Personal Identifying Information (PII) is collected to verify the identity and authorization of individuals requesting access to information in the controlled substance database and to accurately identify the subjects of those requests. The data ensures that only authorized persons obtain sensitive prescription information and that all requests are properly documented for accountability and compliance with state and federal regulations
