DESCRIPTION: In 2008, the Utah Legislature passed Senate Bill 81 which requires individuals working in certain fields to submit verification of U.S. citizenship. The records are used as verification of individual practitioner's citizenship status and as evidence of compliance with the law. Records include a form issued by DRC requesting proof of citizenship. Also included are documents remitted by registrants including name, address, Social Security number, birth date, tax records, and other personally identifying information. If a facility closure occurs, the records of individuals employed there may be transferred to that facility for final disposition.
