DESCRIPTION: These records document information required for an applicant to receive a license as stipulated in Utah Code 58-1-301(1)(b)(ii)(B) and (d) and R156-1-301(4). Information may include Individual Taxpayer Identification Number (ITIN), Alien Registration Number (A-number), unexpired government-issued passport from the applicant's country of residence, and intent-to-hire letter from a Utah-based employer.

-Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home/mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.
