DESCRIPTION: Division applications/files for licensure, registration, or certification are required for all profession types found in the statutes under Title 58. The various Division statutes under Title 58 state that no individual or company may function in a regulated profession or perform duties of a regulated profession unless the individual or company is licensed, registered or certified by the Division. Following receipt for an application for licensure, registration or certification, the Division shall review the application for completeness and whether facts stated in the application are true. For Division profession types that require fingerprints of an applicant, the Division shall compare the fingerprints of the applicant with fingerprints filed with the Bureau of Criminal Identification to determine if the individual fingerprinted has any recorded convictions. This record series may include any of the following: application, fingerprint cards, criminal history, various documents submitted by an applicant, agency correspondence. Division licenses must be renewed according to the renewal schedule identified in Division Rule R156-1. Personal Identifying Information (PII) that may be contained within a Division application: name, username, social security number, date of birth, drivers license number, home address, school transcripts, criminal records, passport number, financial account information, medical records, IP address, and biometric data. Purpose Information is Collected: The Division collects application information to determine if an applicant meets statutory/rule licensure, registration or certification requirements.
