DESCRIPTION: This record series documents the eligibility determination process for licensees referred to or self-reporting to UPHP, as outlined in Title 58, Chapter 4a and Rule R156-4a. This new series includes records for all individuals who undergo the eligibility determination process, but do not subsequently enroll in the UPHP program. Records may include: referral documentation; screening and intake notes; clinical evaluation and treatment reports (including medical, mental health, and substance use disorder records); toxicology results; compliance documentation; case management notes, correspondence (including with external agencies); documentation submitted by the licensee to demonstrate eligibility; and records related to the review of relevant databases (e.g., Utah Controlled Substance Database, criminal history, information related to Division-led investigations).

Personal Identifying Information (PII) that may be included in this record series consists of: name, social security number, date of birth, license number(s), home and mailing addresses, email address, phone number, criminal history, employer details (including name, workplace address, and contact information), emergency contact information (including names, phone numbers, and relationship to the licensee), medical records (including evaluations, treatment plans, progress notes, and prescription information related to medical, mental health, and substance use disorders), biological specimen testing results (including urine, blood, breath, hair, and nail), signatures, handwritten or typed documents, findings from Division-led investigations and information obtained from reviews of relevant databases.

Purposed Information is Collected: UPHP collects information to determine if a licensee meets statutory/rules for entrance into the program.
