DESCRIPTION: Information is collected so that child care providers who accept Department of Workforce Services child care subsidy payments on behalf of customers are paid accurately and efficiently, and child care providers' income can be reported accurately on IRS tax forms.

Information includes payment information regarding children approved for subsidy, and information from child care providers to manage banking and portal accessibility.

Purpose personal data is collected, maintained, or used: Child care providers business contact information and banking information is collected so that subsidy payments can be directly deposited into their account. Child care providers provide their federal tax identification number so that income can be reported accurately on an IRS 1099 form. Customer information is provided so that child care providers can certify the child is enrolled and met minimum attendance requirements and to report any custom rates for customers' child(ren) below the Department's set subsidy rate for the age of the child.

Legal authority for collecting, keeping, or using personal data: Utah Code 35A-3-2-209 Award of child care subsidy services , and CCDF Regulations: 45 CFR 98. The BLS uses a cooperative agreement (CA) to fund cooperative statistical programs because of the agency's ongoing involvement in the programs, pursuant to the Federal Grant and Cooperative Agreement Act of 1977 (31 USC 6301-08).
