TITLE: Quarterly staffing reports

DESCRIPTION: Reports submitted by skilled and intermediate nursing care facilities to show that the facilities have adequate staffing at all times. This report is required in accordance with 42_CFR_431.610(F)(4)_(1994). Includes report on consultants, report on other than nursing personnel, and report on nursing personnel-supplementary page. Information within the reports includes the name and address of the facility, the report date, the report period, the name of the employee or consultant, the employee's position or the consultant's area of consultation, the number of hours worked by the employee or consultant, and a signed certification by the facility administrator that the information submitted is correct and that all standards are being met.
