TITLE: Tooele school district student health records

DESCRIPTION: These are health care screenings and immunization records of school children in the Tooele School District. This program was part of a private grant from the Robert Wood Johnson Foundation which was intended as seed money to start this program. The program was discontinued in 1983.
Included in these records are Student Health Encounter Forms, Complete History and Physical Summary Forms, Communication Forms, and Immunization Update Forms. Information includes student's name, school, identification number, and birthdate; date of visit; student's immunization history, behavioral history, and a review of the student's symptoms; type of visit (initial, follow-up, etc); referral source (parent, teacher, etc); results of any physical examination; results of laboratory tests; treatment; and a record of the immunizations given each student.
