DESCRIPTION: These records are created by the Wildlife Resources Division in order to authorize free fishing privileges for qualified individuals in the custody of the Utah State Training School, the State Hospital, youth detention centers, mental health centers, group homes, halfway houses, or industrial schools as explained in UCA 23-19-14 (1990). The file consists of a three-part form which serves as an application, affidavit, and authorization. One copy goes to the individual, one to the regional office (record series #5239), and one to the Salt Lake office (this record series). The Salt Lake office keeps a separate file for the regional copies.
