DESCRIPTION: This record series is a complete case history of clients receiving services provided in or through a Family Support Office (OFS). Programs are funded by the Division of Family Services (DFS), Services to the Handicapped (DSH), State Office of Family Support(OFS), Aging and Adult Services (DAAS), and/or local discretionary funds. The Human Services client case files include but is not limited to the following types of Eligibility and Determination Forms: Application and Affidavit for Assistance and Food Stamps, Home Energy Assistance Target (HEAT), Reapplication for Assistance and Food Stamps, Application for Financial, Medical, and/or Food Stamps, Application for Social Services, Request for Expedited Services, and Notice of Decision (income report). Copies of: Birth certificates, social security card, car registrations and titles, divorce decrees and other court documents, medical reports, and certification of tribal membership. Payment Forms: Request for Special Need, Handwritten Check Request, Assistance Payments Request, Payee/Case Data Information, Transmittal of Collections, Report on Grant Overpayment, Coupon Books Lost in the Mail, and Forged Check Affidavits. Miscellaneous Documents: Permanency plans/individual service plans, progress notes, Random moment sample survey, activity logs, case activity logs, court orders, lien agreements, and guardianship orders.
