DESCRIPTION: These are patient referrals from hospitals and the Utah Social Services requesting follow-up action by the Tooele County Health Department. They document that requested action was taken. The referrals include letters and Social Service referral forms that request children receive hearing tests and newborn are examined. Each referral was reviewed and specific action was noted on the form. A letter was then sent to the referring agency. This is an outdated file. This method has been discontinued and currently a case file is created for each referral.
