DESCRIPTION: These are case files for children in the Well Child program. This is a low cost program to identify public health problems and to promote healthy children in low income families ineligible for Medicaid. These files are used to document participation in the program and to record the childrens' development. They include the child health record (date admitted, child's name, sex of child, date and place of birth, name and address of parents, name of family doctor or health agency, clinic location, source of referral, prenatal history (weight, length, gestation, delivery, neonatal condition), a record of immunizations, past medical history, psychomotor development history, feeding history, present problems, appraisal of parent, general appraisal, social adaptation and behavior, physical examination, comments, summary and recommendations, physician's signature, nursing note, and nurse's signature); well child care 4-5 years of age form (name, date, age, temperature, weight, height, parent education, history, physical examination, developmental history, and registered nurse's signature); and well child care examination forms (three columns to allow for date, age, weight, temperature, length, head, interval history, died, development, physical examination, problems with plans, other recommendations; immunizations, teaching, next appointment, nurse's signature).

These records are in Archives' permanent custody.

FORMAT MANAGEMENT

Paper: Retain in Office until child age 21 and then destroy.

Paper: Retain in Office for 4 years after child leaves program and then transfer to State Records Center. Retain in State Records Center for 16 years and then destroy.

This disposition is based on advise of the State Bureau of Child Health. This disposition also reflects the previously approved Local Health General Records Retention Schedule (6/87) for Child Health Evaluation Care Screening Evaluation Files. The Well Child and Child Health Evaluation Care Programs (CHEC) are similar programs, the main difference is funding. The CHEC program is federally funded through Medicaid while the Well Child Program is locally funded.
