TITLE: Wildlife duplicate license and permit affidavits

DESCRIPTION: These files document the issuance of a duplicate license or permit as required under UCA 23-19-10 (1990). They consist of a form filled out by a hunter or fisher in order to request a new license when his/her copy has been lost, destroyed, or stolen. Information includes type of license; license number; date of former purchase; amount paid for license; disposition of first license; hunter education number; signature/name of state agent; and signature, address, telephone number, driver's license number, date of birth, sex, and physical description of individual purchasing a duplicate license. Through 2007, there were three copies kept of this form. The yellow copy went to the Division Office and serves as the record copy, the white copy went to the Regional Office, and the pink copy went to Fiscal Management. Currently, the files are kept in electronic format that is accessible by all three offices. This form is also used for license transfer for hunters moving to Utah from another state.
