TITLE: Assistance request files

DESCRIPTION: These requests for assistance files are created by assistance payments workers for the purpose of issuing checks or food stamps to clients. They include the clients name, case number, amount of issuance, amount of food stamps or checks previously issued, the time period the issuance covers, and the signature of the client, case workers, and their supervisors. These records are no longer being created. They have been superseded by the Public Assistance Case Management Information System. This is a computer data base system.

FORMAT MANAGEMENT
Paper: Retain in Office for 1 year and then transfer to State Records Center. Retain in State Records Center for 4 years and then destroy.

See 7 CFR 272.1 1990 for general terms and conditions for states participating in the Federal Food Stamp program. It stipulates that these records should be retained for a minimum of three years and longer when required by state agencies. The Department of Human Services requires a five-year retention because of the activity level of this series.
