DESCRIPTION: This is a record for individuals in each care unit and is separate from the shift overlap. It is a set of general and specific instructions of care, an assessment of needs, and a description of the client's behavioral problems. This information is essential in providing the prescribed care for each client on an individual and group basis. It also describes what precautions an employee must take when caring for certain clients.
Information can be transferred to a new personal book when the resident transfers to a new building.
These personal books include the individual client's name, information sheet, face sheet, photograph, daily schedule, contact record, inservice logs, person centered plan, data sheets, behavioral program, and individual profile. Documents are purged as per data book file content sheet in the front of the book.
