TITLE: Monitoring reports

DESCRIPTION: These are reports that are generated by the activities of the various hospital committees. According to 42 CFR 405, the duties of these various committees also include assuring the quality of professional services provided by the hospital and correcting any problems they may uncover. When a problem turns up, monitoring reports are used to ensure that corrective action is taken.

These reports include the name of the department involved, the problem identified, the date the problem was discovered, the corrective action to be taken, the dates of the monitoring reports, and the patient's age, name, psychiatric information, race, sex, and social security number.
