DESCRIPTION: These are surveys conducted by The Joint Commission on Accreditation of Hospitals and the Centers for Medicare and Medicaid Services to ensure that the hospital is meeting all of the requirements for participation in Medicare/Medicaid. Information includes: the date of the survey, the names of the surveyors, the name and address of the hospital, the department or departments surveyed, the results of the survey, statement of compliance with federal conditions, a statement of deficiencies (if any), and a plan of correction, if necessary.
