DESCRIPTION: These records are created by the audiologist to document student hearing loss, ear infections, and to check hearing aids. The audiologist maintains an administrative copy. The record copy is maintained in the student's academic file, and is eventually microfilmed. These documents are used by students throughout their lives to prove that they are legally deaf. The information is also important to those researching hearing loss and genetics. Included is the child's name, address, birthdate, history of their deafness, fluctuations of hearing loss, speech pathology data, medical records from various institutions, and correspondence to and from doctors and parents.
