DESCRIPTION: These files document the participation of women and children in the WIC program which provides supplemental foods and nutrition education to low income families in accordance with the provision of 7 CFR 246 (1990). This program is open to pregnant, postpartum, and breast-feeding women and to their children under the age of three. These files include nutrition history, ineligibility notification, family enrollment forms, turnabout change order, nutrition education information, nutrition education attendance record, nutrition care plan, pregnancy history, WIC certification form, and WIC participant responsibilities agreement.
