DESCRIPTION: These files are held by the school nurse to document student immunizations and health information required by law and pertinent to the educational setting. The forms are completed by parents and doctors. Information includes student's last physical exam, immunization dates, health problems, hospitalization, handicapping conditions, medical procedures the school needs to perform on the child, allergies, medications, emergency numbers, medical records from hospitals and clinics, dental records, and data on growth patterns.
