DESCRIPTION: This report associates laboratory numbers in the data files with the storet numbers. This report is used when the staff needs to know which records in the data files contain data for a specific storet site. It includes the storet number, the source code, the laboratory code, the laboratory number, the date when the sample was taken, and the time the sample was taken.

This is a working file in the system that is being constantly updated as new information is received. According to the staff, most of the reference to the information is done through the terminals. Printouts are only made upon request. Once the report has been printed out, a 30 day retention is adequate for administrative purposes.
