DESCRIPTION: These are applications of professionals licensed to operate in the State of Utah. The professions involved include podiatrists, dentists, optometrists, pharmacists, accountants, nurses, social workers, and occupational therapists. In all, there are over thirty professions licensed by this division. This activity is required under Title 58 of the Utah Code Annotated. Information includes applications for licenses and registrations, experience certifications, medical certificates, and proof of educational qualifications. This is a general series that in the past has covered several occupations. There are now records series representing each individual occupation. These records contain unique information in rough notes or drafts assembled or created and used to prepare or analyze other documents. The information adds proper understanding to the formulation and execution of policies, decisions, actions, or responsibilities.
