DESCRIPTION: This is a report to the citizens of Utah concerning the personal information held by each State agency and the access to that information. It is published yearly and presented to the Governor and the legislature of the State of Utah. Citizens may obtain a copy from the Archives. This identifies those records series which contain personal information as reported by Utah State Government offices pursuant to the Utah State Information Practices Act (IPA) (UCA 62-3-85.2) and later the Government Records Access and Management Act (GRAMA) (UCA 63-2-101 thru 909) (1991). This report, which is generated from the Archive's local (records management) system, includes agency name and address, responsible authority or chief administrative officer, agency records officer, records series title and description, personal data elements contained in record series, reasons for the collection of personal information on individuals, and designated classification(s).
