DESCRIPTION: This is a chronological listing of materials corresponding reference statistics. It was used for compiling reference and activity reports. It was superseded by the Document Charge-out log. These records include the date of request, the name of agency making request, the records series title, the record date, the file title (which may be an individuals name or social security number), reference statistics for each file, and the number of phone calls received on each day. From 1977 to 1979, a variation of this report was used for similar purposes. It included the date of request, name of agency making request, the number of requests, and the number of phone calls.
