DESCRIPTION: This is a report indicating the number of boxes individual agencies received from the Archives. They are used for billing purposes. This report includes the name of the requesting agency and the respective records officer, the number of boxes delivered, the date, and the signature of the individual receiving the boxes. This report was discontinued when the function of distributing records cartons was turned over to Administrative Services, Central Services, and Central Stores.
