DESCRIPTION: These are records document investigations of possible cross connection drinking water contaminations.
_ Included in these records are incident location, date and time of incident, description of problem reported, date and time of inspection, name of inspector, results of water sample tests, description of actions needed to correct problem, reason for contamination, and the results of follow-up inspections.

According to the discussion with the bureau staff, these records are used by the bureau to ensure that the individuals responsible for the cross connections correct the problem. Failure to do so will lead to administrative actions and, if these are unsuccessful, to the filing of a suit. Administrative actions should be exhausted within a year, at which point suit would be brought. However, due to the lack of experience in this area, an additional year's retention is recommended.
